This is a TTC T1 interface adapter module for use in a FireBERD 6000A communication analyzer/tester. The model number as shown on it is 41440A. This module provides a Tx/Rx pair of bantam jacks, an RJ-45 type RS-232 port and a handset port.
I have read that the FireBERD unit needs to be at rev G or higher to use this module, but cannot confirm this.
