I am not familiar with which model this goes to but as you can see in the below pictures it has 2 cords coming out of it. Both of the plugs are 3 prong plugs. 1 of the plugs has 3 spades and the 2nd plug has 2 spades with a pin coming out of the center of the plug. I have included several pics so you can see exactly what you are purchasing. This might not be the exact one you will recieve but it will be in reletively the same condition. If you purchase this pedal I will put 2 brand new plugs with it if you request. That way if one is ever broken it you will have the plug to replace it!! This is a $50 or more value in itself!!
As you can see in the below pictures these pedals are in good condition. If you have one of the welders that uses this pedal it might be advantageous to have a extra pedal on the shelf in case the idiot you work with runs your pedal over with the forklift!!
